I am 18 years old and I understand that the services/session(s) I will be receiving from Kristi Ottis are not offered as a substitute for professional legal, business, mental health care or medical care and are not intended to diagnose, treat, or cure any mental health or medical conditions. I also understand that Kristi Ottis is not acting as a mental health counselor or a medical professional.

Although Kristi Ottis is a Licensed Professional Counselor by the State of Texas, I understand I am receiving intuitive, mediumship, mentoring, coaching services or sessions and not therapy services.

I also understand that for all legal purposes, any intuitive services provided by Kristi Ottis will be considered for entertainment purposes only.

I understand and agree that I am fully responsible for my well-being during my intuitive, mediumship, mentoring, coaching services or sessions and subsequently, including my choices and decisions thereafter.

I understand that a/an intuitive, mediumship, mentoring, coaching services or sessions are not a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment or medical care, and I will not use it in place of any form of therapy.

I understand that all comments and ideas offered by Kristi Ottis are solely for the purpose of aiding me in achieving my defined goals or outcomes. I have the ability to give my informed consent, and hereby give such consent to Kristi Ottis to assist me in achieving such goals.

I understand that to the extent our work together involves health and wellness or my relationships, Kristi Ottis is not promising outcomes. I also take complete personal responsibility for the choices or interpretations I make as a result of the intuitive, mediumship, mentoring, coaching services or sessions I receive.
